This online application is available from 29 Dec. 2025 to 18 Jan. 2026.

• 如果114學年度第2學期要申請學費分期，請填寫以下表單，生輔組會於2026年1月19日以E-mail寄發空白的分期申請單給有登記的同學，請同學在下學期開學一週內(2026年3月2日)將標示陰影的欄位填寫完畢，給導師、系主任、院長簽名後，統一交到生輔組進行後面的流程。

Please fill this online application if you need to apply for the Instalment for Tuition Fee of the Second Semester of 2025 Academic Year, the applicants will receive a blank file of application form via E-mail on 19 Jan. 2026. The application form need to be printed, and all shadowed boxes are filled in properly, then signed by the class tutor, Head of the Academic Department and the Dean of the Faculty, then handed to the Student Life Section within a week after the opening day of next semester (2nd Mar. 2026) for the rest of the process.

• 待流程完成後，生輔組會將分期後繳費單以E-mail方式寄給同學，請同學務必提供正確的E-mail。

Once the process in completed, applicants will receive installed tuition fee via E-mail, therefore it is required that the applicants provide correct E-mail for online application.

• 如不清楚學費總金額，可先依據前一學期學費總金額填寫表單，實際分期總金額則以校務系統上的金額為主。

Please fill in the amount of the previous semester for online application if you do not know how much the tuition fee of the next semester will be, however, the actual installed amount will be processed according to the amount in the Administration System.

• 住宿費不予分期，請於開學後一週內完成住宿費繳交。

The accommodation fee is not included in instalment, please complete the payment for accommodation fee within a week after the university opening day.
