112學年度第2學期請假注意事項

一、112學年度第2學期學生線上請假系統開放時間為113年2月17日凌晨00:00時至113年6月17日23:59時如本學期請假有誤要修正者,請於113年6月17日前完成。

二、請於請假日七天內完成請假手續。

三、生理假與心理不適假不需上傳證明,事、病假皆須上傳相關證明。病假證明請上傳請假當天、或請假前一天、或請假後一天的就醫收據。手寫藥袋、成藥包裝或藥丸、室友證明、溫度計照片、手機備忘錄截圖不予受理。

四、請於線上請假兩週後,可到校務系統查詢已登錄的請假。查詢路徑為校務資訊系統-教務資訊模組-教務資訊系統-課程資訊-SC0108出缺勤紀錄查詢。(缺曠以SC0108出缺勤紀錄查詢上的資料為主。)

請同學於期末考週時務必上校務系統確認缺曠狀況。

Notes on Leave

1. The Student Leave Application in the 2nd semester of the 2023 academic year is open from 00:00 am on February 17, 2024, to 23:59 am on June 17, 2024. If there is any error on your attendance record of this semester, please complete the correction before June 17, 2024.

2. Please complete the absence application within seven days since the day of your absence.

3. Menstrual leave and psychological discomfort leave do not require proof to be uploaded, but relevant proof must be uploaded for both personal and sick leave. Please upload the medical receipt for the day of leave, the day before leave, or the day after leave as proof of sick leave. Handwritten medication bags, packaging of medicine or pills, proof written by the roommates, photo of thermometer, and screenshots of mobile phone memos will not be accepted.

4. Please check your attendance record in the Administration System after two weeks of taking leave online, . The path is Administration System - Academic Affair Module - Academic Affair System - Course Selection - SC0108 Attendance Record. (The absences will be based on the information in SC0108 Absence Record.)

Please be sure to check the attendance record during the final exam week.

5. Epidemiological leave will be cancelled, and confirmation of diagnosis will be handled in accordance with sick leave regulations.

關於愛校服務

1.依據「實踐大學學生違規銷過實施辦法」第二條:「凡本校學生違犯校規記小過(含)以下之處分,復具悔意且有心改善者,得依本辦法申請違規銷過註銷其懲處記錄。」

2.銷過方式如下: 申誡乙次者:愛校服務四小時;申誡二次者,愛校服務八小時。 小過乙次者:愛校服務十二小時;小過二次者,愛校服務十六小時。

3.本學期之懲處紀錄僅能於本學期辦理銷過,欲辦理銷過之同學,請持違規銷過申請表(點選可下載)至生輔組、宿舍管理室或系上進行愛校服務,服務完畢後請將表單拿到生輔二組銷過,本學期請於113年6月22日前完成服務並將已完成的違規銷過申請表送生輔二組

4.勞作教育不佳乙次者:愛校服務兩小時;勞作教育不佳兩次者:愛校服務四小時,以此類推。

About Campus Services

1. According to Article 2 of the "Implementation Measures for the Cancellation of Student Wrongdoing at Shih Chien University", "Any student who has violated the university regulations and received a minor punishment or less, and who is remorseful and has the intention to repent, may apply for the cancellation of his/her punishment record in accordance with the present Measures."

2. The method of cancellation shall be as follows: If a person is cautioned for one time, it will take four-hour campus service to cancel the caution record; if one is cautioned for the second time, it will take eight-hour campus service to cancel the caution record. The first time the student is admonished for minor punishment, he or she shall carry out campus service of twelve hours. The second time the student is admonished for minor punishment, he or she will be given sixteen hours of campus service.

3. The punishment records for this semester can only be processed for cancellation within the same semester. Students who wish to apply for cancellation should bring the Violation Cancellation Application Form (click to download) to the Student Life Section, the Accommodation Office, or your Academic Department for campus service. After the service is completed, please take the form to the Student Life Section for cancellation. For this semester, please complete the service before June 22, 2024 and send the completed Violation Cancellation Application Form to the Student Life Section.

4. For those who have poor labour education once: two-hour of campus service is required to apply for cancellation; for those who have poor labour education twice: four-hour of campus service is required to apply for cancellation, and so on.
